Volunteer Requirements


Dear Volunteer Candidate,

We are thrilled you are interested in volunteering at Seacoast Hospice! Volunteers provide valuable assistance to our patients, their families and our staff.

Volunteering at Seacoast Hospice requires commitment. As a Medicare-funded agency, the government requires that all Seacoast Hospice volunteers complete all the paperwork included here. Once you have completed the paperwork, please mail the entire packet back to us (it is not necessary to return copies of the Privacy, Security & Safety training documents) and we will contact you for an interview. An interview precedes all training and does not necessarily ensure you will become a Seacoast Hospice volunteer.

Individuals wishing to provide direct care to patients and families are required to attend a 35-hour training. Individuals wishing to provide clerical or indirect assistance are required to attend a four-hour training.

There is a criminal record release authorization form included in this packet. Please do not sign it until you are in front of a notary. You can choose to have it notarized on your own or bring it to Seacoast Hospice to be notarized.

Once completed, please mail all items 1-11below to the Volunteer Manager:

  1. Volunteer application (this may also be completed online here)
  2. Four (4) references
  3. A photocopy of your current driver’s license
  4. A copy of your auto insurance showing effective dates and coverage amounts (example below).

  5. Criminal Record Release signed in front of a notary
  6. Motor Vehicle Authorization Form
  7. Signed Confidentiality Policy
  8. Completed 2-step TB testing (form to be taken to and completed by your physician)
  9. Completed HIPAA Privacy Test and Certification Statement
  10. Completed HIPAA Security Test and Certification Statement
  11. Completed Safety & Compliance Test
