Volunteer Requirements


Dear Volunteer Candidate,

We are thrilled you are interested in volunteering at Seacoast Hospice! Volunteers provide valuable assistance to our patients, their families and our staff.

Volunteering at Seacoast Hospice requires commitment. As a Medicare-funded agency, the government requires that all Seacoast Hospice volunteers complete all the paperwork included here. Once you have completed the paperwork, please mail the entire packet back to us (it is not necessary to return copies of the Privacy, Security & Safety training documents) and we will contact you for an interview. An interview precedes all training and does not necessarily ensure you will become a Seacoast Hospice volunteer.

Once completed, please mail all items 1-4 below to the Volunteer Manager:

  1. Volunteer application (this may also be completed online here)
  2. Two (2) written references (click here)
  3. A photocopy of your current driver’s license
  4. Job Description (click here)

When received the Volunteer Coordinator will contact you to schedule a face to face interview. Following your interview, the following documents will be given and need to be completed prior to the start of training:

  1. Criminal Record Release (Signed by Notary)
  2. Completed 2-step TB testing (Provided by Beacon Hospice)
  3. Physicians Statement
  4. Hepatitis Form
  5. Review Policy Packet and Signature page
  6. HIPPA
  7. Volunteer Agreement

All volunteers wishing to work directly with patients will need to complete a 22 hour training held seasonally at Seacoast Hospice.  For those wishing to volunteer in roles other than with patients shorter trainings are offered.
