 Hospice in the Home

National surveys have shown that the vast majority of Americans wish to remain at home in their last year, surrounded by familiar sights and sounds. Seacoast Hospice helps patients remain at home and supports the family in caregiving. By empowering patients to make informed decisions about their care, we help patients get "back in the driver's seat" and guide them along the road they choose.

Caring for an ill loved one can be physically and emotionally demanding. To help during this challenging time, Seacoast Hospice offers families a reprieve from caregiving and extra support when emotional and physical resources are running low.

We also provide all of our patients with 24-hour-a-day, seven-day-a-week phone support so no matter the time, there is always a trained medical professional available to advise, support, or even visit if needed.
 

Hospice in the Hospital

Some of our patients require hospital services as part of their care plan. For these patients, Seacoast Hospice provides a hospital team that coordinates patient care in the hospital setting. Our specially trained team offers professional support and consulting to the medical staff and nurses and provide a central point of contact for all information regarding the patient's care.

Seacoast Hospice provides hospice care to clients in area hospitals, including but not limited to:

 Hospice in Facilities

For nursing home or assisted living facility residents, Seacoast Hospice can help manage symptoms and control pain through care programs specially designed to complement services already in place. We also help train and support facility staff. Patients receive the advantage of two health care organizations committed to helping patients and families facing end-of-life challenges.

Hyder Family  Hospice House

Most terminally ill people prefer to remain at home, but this is not always possible-especially if the progression of disease causes complications that become difficult to treat in the home.

That's why Seacoast Hospice built the Hyder Family Hospice House — a warm, homelike environment where patients can receive intensive monitoring and 24-hour medical and non-medical care, including spiritual and emotional support.

The 12,700 sq. ft. Hospice House offers 14 individual patient rooms. Just minutes from exit 9 on the Spaulding Turnpike in Dover, New Hampshire, the Hyder Family Hospice House is within easy reach for everyone in the region.

Hospice Care for Children

Seacoast Hospice offers expert pediatric palliative care to children and teenagers with life-limiting illnesses. We help families cope with the specific, emotionally difficult challenges of caring for a sick child by focusing on enhancing the quality of life-for both the child and family.

Together with the family, our care team creates an individual care plan for each child, taking into consideration each family's personal, cultural, and social values.

We provide pain and symptom management as well as age-appropriate therapies, counseling, and support services that address the child's — and the family's-unique needs.
