Early Referrals Improve Quality of Life

After experiencing how much Seacoast Hospice helped them, many of our patients and families say that their biggest regret was not calling sooner. The earlier patients are referred, the more we can do to help.

Anyone can refer a patient to hospice. Individuals can self-refer, or elect to go through their doctors, family members, friends, counselors, nursing home administrators, or hospital discharge planners.

We can immediately meet with the patient and family for a free informational visit to discuss hospice benefits. Even if the patient is not appropriate for hospice, we can identify the best community resources-including our Transitions Program for patients who are not ready for hospice.
