Transitions Program Adapting to the changes and events that accompany a prognosis of a life-limiting illness can be emotional, frustrating, and at times confusing. A person may feel hopeful one day and discouraged the next. It can also be challenging to maintain one's independence while at the same time depend on the assistance of others. Many people don't want to be a "bother" to their family members but do need practical help or have the desire to talk with someone about their illness. Seacoast Hospice realizes that asking for help may be difficult for many people, and that it may take some courage to do so. But once you call us, we take the time to sit down with you and personally assess your specific needs. We then develop a plan of care that includes information or referrals pertaining to area services providers, local support groups, or online resources. Additional services include monthly visits or phone check-ins, assistance with advance care planning, and connecting patients and families with trained volunteers.
